Closure of Cascade Home Health Program

After a thoughtful and comprehensive review of all Cascade Health service lines, we have made the difficult decision to close our Home Health program. PeaceHealth, a key community partner, is expanding the capacity of its Home Health services to help meet the continued needs of Lane County.

This decision, while necessary for the long-term alignment of our organization, in no way reflects the dedication or performance of our Home Health team. We are deeply grateful for their compassion, professionalism, and the exceptional care they have provided to patients and families throughout our community.

What This Means for Current and Future Care

Our highest priority is ensuring a smooth and uninterrupted transition of care for patients. To support this process:

  • Current Home Health patients may choose to continue care with PeaceHealth or select any other licensed Home Health agency that best meets their needs.

  • We will work closely with each patient’s chosen provider to ensure a seamless transition of services.

  • All other Cascade Health programs—including Hospice Services and the Pete Moore Hospice House—remain fully operational, continue to grow, and are committed to serving our community.
