Physical Therapist

Responsible to: Home Health Clinical Manager              
Hours: 32 - 40 hours/ week
Schedule: Monday - Friday 8 hours/ day
Pay Range: $42.92 - 64.38

Job Summary:

The Physical Therapist will function as a member of the home care team, providing services in the patient’s place of residence in accordance with physician orders. Evaluate, treat, and instruct patients to assess, prevent, correct, alleviate, and limit the signs and symptoms of physical disability, bodily malfunction, and pain. Perform tests and measurements to evaluate patient functionality and create a customized treatment program that includes the use of physical measures, activities, and devices for preventative and therapeutic purposes. Provide consultative, educational, and other advisory services to reduce the incidence and severity of physical disability, bodily malfunction, and pain.

Education & License Requirements:

Current licensure in the State of Oregon as a Physical Therapist is required.

Minimum Qualifications & Experience:

One year of inpatient or outpatient physical therapist experience is required. Must have a current driver’s license, an insured vehicle for transportation, and a means of communication. 

Essential Job Functions:

  • Evaluate patients, plan, and implement effective therapeutic interventions based on physical assessment, clinical judgment, and psychological sensitivity. 

  • Understand the hospice philosophy and may provide care to dying patients and their caregivers.

  • Utilize current standards of practice and follow departmental policies and procedures.

  • Comply with applicable accreditation and regulatory requirements governing the practice of physical therapy. 

  • Maintain a high level of professional expertise and assume responsibility for professional growth and development. 

  • Provide supervision to PTA’s and PT students assigned to the department, when appropriate, and participate in the orientation of new employees.

  • Collaborate with other home care services, including nursing, social work, and additional therapies, to provide patient care.

  • Administer treatments and use therapeutic equipment; document treatment utilizing software system.

  • Educate patients and their caregivers.

  • Work independently and be adaptable to change.

  • Demonstrate sound judgment and effective time management skills.

  • Meet safety needs of patient.

  • Communicate clearly, including verbal, written, telephonic, and electronic.

  • Participate in department improvement programs, conferences, staff meetings, and in-services.

  • Develop good working relationships with referral sources.

  • Negotiate terrain and physical obstacles to reach patients.

  • Take vital signs, move, and transfer patients.

  • Organize, prioritize, and delegate at a fast pace.

  • Perform CPR and respond to emergencies.

Marginal Job Functions:

Answer phones and perform various clerical duties. Move furniture, equipment, and supplies. Light mechanics and instrument maintenance. Post and access schedule. Perform other duties as assigned.
