Responsible to: Senior Home Care Manager
Hours: 32 - 40 per week, 8 hours / day
Schedule: Varies
Pay Range: $41.19 – 63.50
Job Summary:
The registered nurse functions as a member of the Home Care Services team, providing skilled care to Home Health and Hospice patients in their place of residence. All care is given in accordance with physician’s orders. Evaluate patients, plan and implement effective therapeutic interventions based on physical assessment, sound judgment, and psychological sensitivity. Understand the Hospice philosophy and provide treatment to dying patients. Utilize current standards of practice and follow departmental policies and procedures. Comply with applicable accreditation and regulatory requirements governing the practice of nursing. Maintain a high level of professional expertise and assume responsibility for professional growth and development. When appropriate, provide supervision to the Home Care Aide and Hospice Aide assigned to the patient. Assist with admissions with the ability to assist with referrals department in processing new referrals, meets with patients and families to explain the hospice benefit and facilitate their transfer onto hospice services. Doing audits when assigned Participate in the orientation of new employees, perform audits when assigned, and take student nurses to observe as assigned
Education & License Requirements:
Current licensure in the State of Oregon as a Registered Nurse and valid CPR certification.
Minimum Qualifications & Experience:
Three years of recent (within the last five years) med/surg experience, or one year of (within the last three years) critical care experience. Needs to be comfortable with computers and able to document using a laptop computer. Must have a dependable, insured vehicle for transportation to visits, a current driver’s license and an insurable driving record, and a means of communication. Skills include: a high degree of written and verbal communication, flexible, team player. Good case management, time management, and assessment skills.
Essential Job Functions:

Deliver patient and family education.

Use computers to document patient records.

Participate in conferences, staff meetings, and in-services.
Organize, prioritize, and delegate work as appropriate.
Move and transfer patients.
Use health care related equipment.
Develop a good working relationship with referral sources.
Perform CPR and respond to emergencies.
Negotiate terrain and physical obstacles to reach patients.
Drive an automobile safely.
Share on-call responsibilities.
Marginal Job Functions:
Manage central lines – including IV therapy. Obtain lab specimens. Answer phones, perform clerical duties, put together charts, move furniture, equipment, and supplies. Post and access schedule. Light mechanics, instrument maintenance.
