Clinical Liaison Staff RN
Responsible to: Pete Moore Hospice House Manager
Hours: 56 hours per pay period, 8 hours/day
Schedule: Every other week, Wednesday - Tuesday
Pay Range: $41.19 - 63.50
Job Summary:
The Clinical Liaison Staff RN functions as a member of the hospice care team, serving as a liaison between the Hospice and Pete Moore Hospice House departments and the hospitals, physician offices, and community referral sources to facilitate admissions and to educate patients, families, physicians, and physician office staff regarding available services. Generates and processes appropriate patient referrals/admissions from within our service area. Establishes and maintains strong, positive interpersonal relationships with staff in community entities, including physician offices, nursing homes, assisted living facilities, and hospitals. Interactions will typically include, but are not limited to, Case Managers, Discharge Planners, Patient Care Managers, Directors of Nursing, and other community referral sources. Visits hospitalized patients who have been referred to Cascade Hospice to facilitate admission to Hospice and Pete Moore Hospice House services and to complete Election of Benefit paperwork for new patients. This position will also provide backup for the Referrals RN tasks. The Clinical Liaison Staff RN will disseminate relevant information to staff, and secure initial and ongoing insurance authorizations when necessary. Must understand the hospice philosophy, utilize current departmental standards of practice, and follow organizational policies and procedures. Provides excellent customer service. The Clinical Liaison must be dedicated to the mission and core values of Cascade Health.
Education & License Requirements:
Current licensure in the State of Oregon as a Registered Nurse and valid CPR certification. Obtain food handlers permit upon hire.
Minimum Qualifications & Experience:
Two years of recent (within the last five years) acute care experience, or one year (within the last three years) critical care or oncology experience. Two years Home Health or Hospice experience preferred. Need to be comfortable with computers and able to document using a laptop computer. Desired skills include: High degree of written and verbal communication skills, flexible team player, clinical excellence, and triage skills.stock supplies in patient rooms
Essential Job Functions:
Evaluate patients, assess pain and symptom management needs.
Develop nursing plan of care, interventions, and assure continuous re-evaluation.
Maintain responsibility for patient’s care management from admission to discharge.
Work in close collaboration with the interdisciplinary team and referring physicians.
Administer treatments, including IV, respiratory, enteral therapy, and phlebotomy.
Provide wound care and maintain catheter drainage systems.
Manage medication administration.
Move, transfer patients.
Provide personal care, hygiene, and post-mortem care.
Deliver patient and family education.
Work independently in an organized manner utilizing good time management skills.
Complete electronic clinical documentation within required timelines.
Demonstrate sound judgment and communicate clearly – verbal, written, telephone, and electronic.
Participate in conferences, staff meetings, and in-services.
Use health care related equipment: patient lift device, gastric and respiratory suction, and oxygen systems.
Develop a good working relationship with referral sources.
Perform CPR, respond to emergencies.
Marginal Job Functions:
Answer phones, perform clerical duties, stock supplies in patient rooms, light cooking, stock supplies in patient rooms.
