Here, you will find answers to the most frequently asked questions about our home health services.

  • Who is eligible for home health services?

    Anyone 65 years or older and has Medicare.

    Anyone under the age of 65 who is disabled and has private insurance.

    When an acute episode of care requires hospitalization, home care serves as the intervention before hospitalization, and the care following.

    You may be eligible for home health services if you meet the following requirements:

    • You must be under the care of a doctor and receiving services under a plan of care that's established and reviewed regularly by a doctor.
    • You must need, and a doctor must certify that you need, one or more of the following:
    • Intermittent skilled nursing care
    • Physical therapy
    • Speech-language services
    • Occupational therapy
    • The home health agency caring for you must be approved by Medicare -Cascade Home Health is Medicare Certified.
    • You must be home-bound, and a doctor must certify that you're home-bound. To be home-bound means:
    • Your condition keeps you from leaving home without help, such as using a wheelchair or walker, needing special transportation, or getting help from another person.
    • It takes a considerable and taxing effort to leave home.
    • Only leaving home for medical treatment or short, infrequent absences for non-medical reasons, such as attending religious services and hair appointments. You can still get home health care if you attend adult day care, but home care services would be delivered at home.
  • Who pays for home health services?

    Home health services are paid by Medicare, Medicaid/Oregon Medical Assistance Plan, managed care organizations, private insurance or private payers.

  • How often will home health staff visit me?

    The number and frequency of visits will be based upon your physician's orders and your individual plan of care.

  • I'm being discharged from the hospital. Can I choose my home health provider?

    Yes, patients have the right to choose their provider. You can specifically ask for Cascade Health Solutions to serve as you home health provider, regardless of whom your primary care provider might be, or what hospital or skilled nursing facility you've been treated in.

  • Is Cascade Home Health staff wound care certified?

    Yes. All of our case managers are KCI Wound V.A.C.® Certified.

  • Is Cascade Home Health Medicare certified?

    Yes. We are Medicare Certified.

  • How long has Cascade Home Health provided services?

    We have provided home health services locally and in Lane County for more than 30 years.

  • How do I make a referral?

    Making a referral to Cascade Home Health is easy. Simply call 541-228-3050 and provide the patient's name, date of birth and demographic information.

  • What happens after a referral is made?

    We will call you to schedule an initial visit to have a nurse assess the patient's condition and needs. Our clinician will then contact your medical office to report the findings and suggest a plan of care. Subsequently, we'll contact you with any change in status, complications, or problems achieving set goals. The number of visits is based on the physician's orders and individual needs. Skilled medical services are covered by Medicare and most other insurance plans.
