Job Summary: Participates within a multidisciplinary team who treats and manages health conditions. Provides clinical services within the scope of nursing practice. In working with those with diabetes, follows the American Diabetes Association Clinical Practice Guidelines in providing recommendations in management and treatment of diabetes. Motivates and facilitates lifestyle change with particular emphasis on self-management and prevention of chronic health conditions in individuals and groups. Develops and maintains the clinical/nursing component of CHL programs. Provides assessment and treatment plans to those referred. Documents and provides communication to referring PCP/specialist. Develops written material for use in classes, workbooks, and individual counseling. Develops and provides self-management and prevention education (workshops, classes, and individual appointments) as needed for CHS needs; including presentations for community health care partners. Provides clinical services (blood pressure, weight, blood glucose, pulse, injections). Participates in case management. Participates in special projects when assigned.

Job Summary: The Medical Social Worker functions as an active member of the hospice interdisciplinary team and renders services in accordance with the plan of care in the patient's place of residence. All care is given in accordance with physician's orders. Evaluates patients, plans and implements effective therapeutic interventions based on assessment, sound judgment, and psychological sensitivity. Understands the hospice philosophy and provides care to dying patients. Utilizes current standards of practice and follows departmental policies and procedures. Complies with applicable accreditation and regulatory requirements governing the practice of social work. Maintains a high level of professional expertise and assumes responsibility for professional growth and development. When appropriate, the Medical Social Worker provides supervision to students assigned to the department. Participates in the orientation of new employees.
