Overview

Cascade Health Solutions is pleased to announce the opening of a new Primary Care Clinic to provide affordable membership-based primary medical care for small businesses, families, and individuals who may not have access to adequate health care.  
 
Primary health care focuses on promoting health and preventing illness through health education and counseling, screenings, access to treatment services, and chronic disease management for a variety of conditions, including diabetes and high blood pressure.
 
This program is not an insurance plan; rather, individuals choose to become a member of this cost-effective Primary Care Clinic. The clinic is staffed by a licensed Family Nurse Practitioner and is designed to provide members easy access to quality primary health care, including preventive services, and mental health services.
 
The opportunity to enroll is available to any person in Lane County, except those who are Medicare eligible or currently covered by Medicare.  Membership is on a month-to-month basis and is limited to the services described in the "Member Handbook".

Cost of Services

The Primary Care Clinic will be available to you for appointments and treatment on the first day of the calendar month following completion of the enrollment process.  This process includes submitting a completed enrollment form and payment of the first month's $45.00 membership fee for each enrolled member when there are up to three (3) members in a household.   For households of four (4) or more members, the membership fee is $40.00 per month for each enrolled member.  In addition to the monthly membership fee, there is a per visit office fee of $20.00.  There is no fee to enroll; however, if a member cancels his or her membership and then re-enrolls, there is a $99.00 charge to re-enroll.  We accept cash, check, or Visa/Mastercard/Debit card for the initial membership fee.  The ongoing monthly membership fee will be processed through electronic funds transfer (EFT) from a checking/saving account or through a credit/debit card transaction.
