Diabetes Self-Management
Nationally recognized by the American Diabetes Association, the Center for Healthy Living, helps people living with diabetes understand all aspects of the disease and how to control it.
We work with patients to identify ways to manage diabetes, such as fitting more activities into the day or making positive behavior changes to meet specific goals.
After receiving a diabetes health assessment to determine current health and special needs, patients typically make three to six visits to the program for one-on-one or group meetings.
With a physician's referral, most insurance plans (including Medicare) will cover part or all program costs.
Diabetes Prevention - A guide to better health
This class and/or individual session with our trained staff is intended for anyone who has been diagnosed with pre-diabetes, glucose intolerance, insulin resistance or metabolic syndrome. It is designed to help you make important lifestyle changes in diet and exercise to prevent or delay the onset of diabetes and decrease your risk for cardiovascular disease. Emphasis will be on starting an exercise program, eating healthy for life and goal setting.
Qualified Staff
A qualified, multidisciplinary team of professionals comprised of registered nurses, registered dieticians, and certified diabetes educators provides services at the Center for Health Living. A certified pump trainer assists those going on an insulin pump or requiring adjustments to their therapy.
